Chronic condition prevention and management

Vida Health is a virtual health program. It offers personalized coaching and resources to help you take steps to prevent chronic conditions and related health issues or manage a chronic condition.

Prevent: Dedicated health coaching and tracking

The Preventive Program focuses on prevention of chronic conditions, as well as building healthy habits, with the support of a non-clinical health coach – all at no cost to you. Meet virtually with a health coach on a regular cadence and stay connected through in-app or desktop messaging. Your coach will help you identify personal health goals to reduce your risk for diabetes and other chronic conditions by helping you:

• Reach or maintain a healthy weight
• Improve your nutrition including support for special diets (i.e., gluten-free, paleo, vegan)
• Exercise more and get stronger
• Lower stress and improve sleep.

Those who are not enrolled in an Edward Jones medical plan but have a chronic condition are eligible to participate in the Resilience Program from Vida Health, which helps build resilience through positive thinking, self‑care, social connection, mindfulness, purpose and emotional regulation.

Online support groups and additional resources can boost your success and help you stay informed about a wide variety of well-being topics. You can track progress toward your goals by connecting Vida Health with a fitness app, such as Fitbit, Apple Health, Garmin and many others.

All active U.S. associates, regardless of medical enrollment, as well as U.S. spouses, domestic partners and dependents age 18 or older who are medically enrolled (defined as enrollment in the Anthem Gold or Silver medical plan or the HMSA medical plan) are eligible to participate in Vida Health’s Preventive Program at no cost.

Manage: One-on-one help from medical professionals

The Chronic Conditions Program helps you manage chronic conditions such as diabetes, hypertension or high cholesterol.

You can access Vida Health’s care team and helpful resources through the desktop or mobile app. Depending on your condition, your program may include:

• One-on-one coaching with dietitians, nurses and nurse practitioners
• Help with labs, prescriptions and clinical monitoring
• Devices like scales, blood pressure cuffs, glucometers and continuous glucose monitoring devices

All medically enrolled U.S. associates, spouses, domestic partners and dependents age 18 or older are eligible to participate in Vida Health’s Chronic Conditions Program at no cost. For purposes of eligibility, “medically enrolled” refers to enrollment in the Anthem Gold or Silver medical plan or the HMSA medical plan, whether through active coverage or COBRA continuation.
